Berwick Surgicentre is an accredited Day Surgery serving the needs of the Berwick and its surrounding suburbs. It is contracted with all health funds , operates Saturdays and has  provided modern , safe ophthalmic surgery for over 13 years

Your doctor will book your procedure with us and will provide you with the ‘Patient Registration’ form, for you to read, complete and return to Berwick Surgicentre.  

Alternatively the form can be downloaded from our website.  It is important that we receive your form at least 72 hours before the day of your procedure, so that we can advise your admission information including financial consent and the time of your procedure. 

 Please ensure that if you have a mobile phone that your number is included on the Patient Registration form.   

Fax your completed Patient Registration form to (03) 97689211  or deliver to Berwick Surgicentre at 22 LANGMORE LANE BERWICK 3806.

The completed form can also be returned via mail to the above address

On the Day of Surgery   

Pre-admission fasting   On the morning of your surgery, you will need to fast for at least 6 hours prior to surgery.  It is important you follow these instructions to prepare you for the general anesthesia associated with your procedure.   What to wear   For your convenience we suggest you:   Shower prior to arriving at Berwick Surgicentre

Do not wear makeup, nail polish or any jewellery (wedding ring acceptable)   Wear loose comfortable clothing and low heeled shoes   What to bring   In order for us to complete your patient admission, please bring with you:   your Medicare card, DVA card if applicable, as well as your private health insurance card or details with you.

Please bring your preferred method of payment as you will need to settle any gap costs at this time.   a list of all prescribed medications, plus the names of any over the  counter medications you are taking   If you wear contact lenses they will need to be removed before surgery so we suggest you bring your lens case.   

For your security reasons, we recommend you leave any valuables at home.   

Berwick Surgicentre is a totally smoke-free environment in line with Victorian Health Department Legislation ,  


Accompaniment by partner, friend or relative   You will need to arrange to have an adult collect you after your procedure and continue to have an adult with you for the remainder of the day and overnight.  

For safety reasons following the use of anaesthetic in your procedure, Berwick Surgicentre does not allow patients to travel unescorted.   Due to health department regulations, it is not possible for your partner, friend or relative to be with you throughout the procedure in the theatre. 

While your procedure is being performed, your companion is welcome to wait in our   reception area.  Refreshments are available  by a vending machine   They will be contacted by the DAY SURGERY when you are ready for discharge.   When you are discharged you will be given instructions regarding post surgery care.
